What is pre-registration?

Pre-registration is now available in Waterloo region. By pre-registering you will be providing information that will allow us to swiftly book appoints as vaccination doses become available. Pre-registering is the fastest way to get a COVID-19 vaccine appointment.

We ask those patients who are located outside of Waterloo Region to check eligibility criteria specific to their location.

Information Required for Pre-registration

1. Contact information including name and full address

2. Cell/mobile phone number, or home phone number

3. Email address

4. Priority group information

Notice of Collection: Personal information is collected under the authority of s. 5(2) of the Health Protection and Promotion Act. This information will be used for the control of disease and the provision of immunization services, in compliance with legal and regulatory requirements. Any questions about the collection of personal information should be addressed to the Service First Call Centre (519) 575-4400.

What Happens After I Pre-register?

When COVID-19 vaccine is available for your priority group, you will be contacted through the method you selected (email, text, or phone call). Please ensure you check your JUNK/SPAM folder in your email. 

On the day of your appointment please bring identification that supports your role in the eligible priority group. This can be a workplace identification badge, proof of employment letter, or a personal letter from your employer.
