COVID-19 Pre-registration information

What is pre-registration?

Pre-registration is now available to priority groups in Waterloo region. By pre-registering you will be providing information that will allow us to swiftly book appoints as vaccination doses become available. Pre-registering is the fastest way to get a COVID-19 vaccine appointment. 

Due to limited supply COVID-19 vaccination appointments are not available to the general public. Your eligibility will be confirmed before any appointment is booked. 

Who can pre-register?

Anyone who falls into the following groups is invited to pre-register for a COVID-19 vaccination appointment:

1. Long-term care and retirement home staff, residents and essential caregivers

2. Seniors group setting staff and residents

3. Adults 60 years of age or older

4. Indigenous adults 

5. Adults receiving chronic home care


  • Adult recipients of chronic home care

  • Adults 80 years of age and older (born in 1941 or earlier)

  • Health care workers

  • Indigenous adults

  • Long-term care and retirement homes staff and essential caregivers

  • Seniors group settings staff, residents, and essential caregivers


  • Adults 60 years of age and older (born anytime before or during 1961)

  • Adults with highest-risk health conditions:

    • Organ transplant recipients​

    • Hematopoietic stem cell transplant recipients

    • Individuals with neurological diseases in which respiratory function is compromised 

      • Motor neuron disease, myasthenia gravis, multiple sclerosis​

    • Haematological malignancy diagnosed less than one year ago

    • Kidney disease (eGFR less than 30)

  • Adults with high risk health conditions

    • Obesity (Body mass index > 40)​

    • On treatment causing immunosuppression

      • Chemotherapy, immunity-weakening medications​

    • Intellectual or developmental disabilities 

      • Down syndrome, autism spectrum disorder, fetal alcohol spectrum disorder, cerebral palsy​

  • Essential caregiver for an adult or child with a high risk condition who requires support for daily living

  • Faith leaders who are in close contact with persons and families in the following circumstances

    • End-of-life care​

    • Care of the deceased, funerals, bathing, or other ceremony with direct contact with decease persons

    • Home visits to high risk individuals

    • Pastoral care in hospitals, long-term care, or retirement homes

  • Residents and frontline staff in the following high risk settings

    • Supportive housing​

    • Developmental services

    • Emergency homeless shelters

    • Other homeless populations not in shelters

    • Mental health and addiction congregate settings

    • Homes for special care

    • Employer-provided housing for farm workers, including temporary foreign workers

    • Adult correctional facilities 

    • Violence against women shelters 

    • Anti-Human trafficking residences

    • Children's residential facilities, including foster homes

    • Indigenous healing and wellness

    • Youth justice facilities

    • Bail beds

  • One essential caregiver for residents in the following high-risk congregate settings

    • Developmental services​

    • Mental health and addictions congregate settings, including supportive housing

    • Homes for special care

    • Children's residential facilities, including foster homes

    • Indigenous healing and wellness

2. Cell/mobile phone number, or home phone number

3. Email address

4. Priority group information

Notice of Collection: Personal information is collected under the authority of s. 5(2) of the Health Protection and Promotion Act. This information will be used for the control of disease and the provision of immunization services, in compliance with legal and regulatory requirements. What Happens After I Pre-register?

When COVID-19 vaccine is available for your priority group, you will be contacted through the method you selected (email, text, or phone call). Please ensure you check your JUNK/SPAM folder in your email. 

On the day of your appointment please bring identification that supports your role in the eligible priority group. This can be a workplace identification badge, proof of employment letter, or a personal letter from your employer.
